Application Process

  1. Download the application form (PDF) >
  2. Print, complete, and sign form.
  3. Mail form along with a non-refundable application fee of $50 (check made payable to MMA ) to:

  4. After receipt and review of your application, an education director will contact you.

Application Fee
Non-refundable, $50 fee for each applicant.

Tuition Deposits
Non-refundable and non-transferable. Deposits are due with the signed contract.
Deposit will cover first month tuition fee if paying monthly.

Payments
Make checks payable to MMA. Payments are due on the first of each month.
